Massachusetts requires any operator involved in a motor vehicle accident in which a person was killed, a person was injured, or there was damage in excess of $1,000 to any vehicle or property to complete a Motor Vehicle Crash Operator Report.
Use the information you received at the time of the accident (the information exchange form an officer may have filled out for you, photos of the other license plates/driver’s licenses, etc.) to complete your Operator Report to the best of your ability. Your Operator Report should be completed within 5 days of the accident. Complete instructions can be found on the first page of the form.
You will need to submit your completed Operator Report to 3 places:
- Drop off / mail / email one copy to the Northborough Police Department (email to dispatch@town.northborough.ma.us)
- Submit one copy to your insurance company
- Mail one copy to the RMV:

Not all accidents require a report be written by a responding officer, nor does the responding officer determine fault. For example, minor accidents on private property (parking lot of a retail store, a private way, etc.) will not generate a police report. In most instances, your completed Operator Report is all your insurance company will need to proceed with the claims process. In the event you do need to request a copy of an accident report, you may do so here. (Note – when requesting a report through the LexisNexis site, the last name entered must match the last name of either the operator or registered owner of the vehicle involved. It will prompt you for credit card information, however there is no charge to request the report for an accident that occurred in Northborough)
